School Immunizations

Childhood Immunizations Requirements
School Year 2010-2011
Florida Certificate of Immunization, Form DH 680, is the only document that the schools are permitted to accept as proof of immunization.
Shots may be obtained through your local physician or Palm Beach County Health Department Health Centers or Palm Beach Health Department Immunization Van located throughout the county.

Required
Diphtheria/Tetanus/Pertussis (DTaP)
Polio Series
Mumps/Measles/Rubella (MMR)
Hepatitis B Series (Hep B)
Varicella
Pneumococcal - children 2-24 months entering childcare
Tetanus/Pertussis Booster (Tdap) 7th grade

    By the time a child starts school he/she should have already had a number of required immunizations. However, proof of updates, boosters, and series completion is required to enter Palm Beach County Schools for the first time and to advance to some grade levels. A physical is required for entrance to Pre-kindergarten, kindergarten and those entering 7th grade.
Proof of the following are Required for New Students to Register, Transfer Students And Entry To Some Grade Levels.
Diphtheria/Tetanus/Pertussis

(4 if last dose given after age 4)
(REQUIRED  GRADES PRE- K -12)
Polio Series
4 doses
(3 if last dose given after age 4)
(REQUIRED  GRADES PRE- K -12)

2 doses MMR, (Mumps, Measles, Rubella)
(
on or after 1st birthday)
May also be given as 2 doses of MMRV (Varicella)
(REQUIRED GRADES PRE-K -12)
Hepatitis B (Hep B)
3 doses
(REQUIRED FOR GRADES PRE-K-12)
Tetanus/Diphtheria/Pertussis Booster (Tdap)
1 dose Tdap (REQUIRED GRADE 7)
proof of temporary medical exemption if Tetanus booster Td given in last 2-5 yearss
Varicella (Chicken Pox)
1 dose (or provider documentation of chickenpox)
(REQUIRED GRADES 3-9)
 

2 dose (or provider documentation of chickenpox)
(REQUIRED FOR KINDERGARTEN - SECOND GRADE)
